What is a Home Care Package?
The Home Care Packages Program is an Australian government funded program designed to help older people live safely and independently at home for longer.
Upon receiving a Home Care Package, the government provides funding for your care, which is directed to the Home Care Package provider of your choice.
There are four levels of funding for Home Care Packages, from basic care needs (Level 1) through to high care needs (Level 4). These packages are consumer-directed, which means you have control over the types of care and services you receive. They can be used to receive all kinds of support, such as personal care, help with housework, transport to appointments or nursing and allied health services.
Who is Eligible for a Home Care Package?
Home Care Packages are designed to support seniors over 65 (or over 50 for Indigenous Australians) who wish to stay in their homes rather than move into assisted living or nursing homes.
An online eligibility checker is available on the My Aged Care website to help you determine whether you meet the requirements. You may be eligible if you've noticed changes in your abilities, have a medical condition or reduced mobility, experienced shifts in family care arrangements, or recently had a fall or hospital admission.
To access a Home Care Package, you must first be assessed by the Aged Care Assessment Team (ACAT). The ACAT are a team of medical, nursing and allied health professionals who are specifically trained to evaluate individuals with complex needs who may require aged care services.

How Does Home Care Package Occupational Therapy Work?
Once you are approved for a Home Care Package (HCP), you’ll need to sign up with a HCP provider. HCP providers are 3rd party companies that manage their clients funds on their behalf. If you would like to work with us just let us know and we’ll get in contact with your case manager to introduce ourselves. Alternatively, your case manager may refer to us directly.
Referral: Once we’ve received a referral we will call our you or your HCP manager within 24 hours to introduce ourselves and organise a time for an initial consultation with our occupational therapist.
First Appointment: Our occupational therapist will visit you at home for the first appointment and listen carefully to your concerns, working together to establish your goals. If you require home modifications or assistive technology your occupational therapists may take measurements and begin planning at this appointment. If your goals are related to ongoing therapy, your occupational therapist will chat with you and your loved ones to make a plan based on your goals.
Second Appointment: Depending on your goals, at this appointment your occupational therapist may trial assistive technology with you, or continue their assessment, which involves gathering subjective information through a chat with you or a support person, and objective assessment through a range of outcome measures and observation.
Reports: Your occupational therapist will go away and do their paperwork! This may entail making recommendations regarding assistive technology or home modifications. If your goals are related to ongoing therapy, your occupational therapist will create a health plan detailing how they plan to work with you to achieve your goals. They will share this with you and your HCP manager.
Checking in: If necessary we will see you again to make adjustments to your assistive technology so that its suitable for your use, or teach you how to use it. If you are undertaking ongoing therapy we will regularly monitor and assess your progress, making adjustments to the care plan as needed.

The number of sessions depends on your goals. For example, if you require an off the shelf assistive technology item such as adaptive cutlery you may only require an initial assessment. Whereas if you require more complex equipment you will need to trial it before purchasing to ensure it suits your needs, and your occupational therapist may need to do a follow up assessment once it arrives to make sure it’s working for you, and give you some training if required.
