What is a Home Care Package?

The Home Care Packages Program is an Australian government funded program designed to help older people live safely and independently at home for longer.

Upon receiving a Home Care Package, the government provides funding for your care, which is directed to the Home Care Package provider of your choice.

There are four levels of funding for Home Care Packages, from basic care needs (Level 1) through to high care needs (Level 4). These packages are consumer-directed, which means you have control over the types of care and services you receive. They can be used to receive all kinds of support, such as personal care, help with housework, transport to appointments or nursing and allied health services.

Who is Eligible for a Home Care Package?

Home Care Packages are designed to support seniors over 65 (or over 50 for Indigenous Australians) who wish to stay in their homes rather than move into assisted living or nursing homes.

An online eligibility checker is available on the My Aged Care website to help you determine whether you meet the requirements. You may be eligible if you've noticed changes in your abilities, have a medical condition or reduced mobility, experienced shifts in family care arrangements, or recently had a fall or hospital admission.

To access a Home Care Package, you must first be assessed by the Aged Care Assessment Team (ACAT). The ACAT are a team of medical, nursing and allied health professionals who are specifically trained to evaluate individuals with complex needs who may require aged care services.

Home Care Packages (HCP) can be used for physiotherapy to keep you active and independent in your later years. This may look like one-on-one therapy with a physiotherapist or physiotherapist-led group exercise or hydrotherapy classes. Services may occur at home, or at a clinic.

They will thoroughly assess your mobility and determine whether you may benefit from a gait aid such as a walking stick or walking frame. They will measure and fit the aid to your individual needs and teach how to safely use it.

  • Improving your balance can greater your capacity to transfer and mobilise, ultimately resulting in more independence in the home and community, and reduced falls. We work with you to improve your balance whilst implementing strategies to ensure your home environment is safe and the risk of falls is reduced.

  • Our physiotherapists incorporate strength training to build strength, improve stability, and enhance mobility. We conduct thorough assessments of your strength so that we can tailor exercises to your individual needs. We create home exercise programs you can complete in our clinic or in the comfort of your own home.

  • Our physiotherapists focus on enhancing cardiorespiratory fitness and endurance through targeted exercises. By strengthening heart and lung function, they help individuals improve aerobic capacity, respiratory strength, and overall endurance. This supports clients in managing health conditions effectively, improving their capacity to complete daily activities.

  • Our physiotherapists provide evidence-based guidance to help manage pain. This may include hands-on treatment, advice regarding ways to self-manage your symptoms and the formulation of a plan to get back to activities you enjoy. We can also assess for pain relief assistive technologies such as a TENS machines and provide education so that they are implemented effectively and safely.

  • Our physiotherapists provide specialised rehabilitation for people with a wide variety of diagnosis. This includes conditions such as spinal cord injury, stroke, acquired brain injury, Parkinson’s disease, Motor Neurone Disease, Multiple Sclerosis, chromosomal disorders, intellectual disabilities, and amputees.     Our experienced physiotherapists offer one-on-one hydrotherapy in three convenient locations in Adelaide: Parafield Gardens, Campbelltown and Seaton. However, we are often able to provide services at a pool local to you.

  • We can assess your ability to transfer and mobilise and develop a manual handling plan. This plan can be used to guide your formal or informal supports so that they can best support your daily needs. We also offer manual handling training.

  • Assistive technology is any specialised equipment that enhances the learning, working and daily living for people. Our physiotherapists work with you to determine whether you may benefit from any assistive technology such as gait or transfer aids.

Once you are approved for a Home Care Package (HCP), you’ll need to sign up with a HCP provider. HCP providers are 3rd party companies that manage their clients funds on their behalf. If you would like to work with us just let us know and we’ll get in contact with your case manager to introduce ourselves. Alternatively, your case manager may refer to us directly.

  1. Referral: Once we’ve received a referral we will call our you or your HCP manager within 24 hours to introduce ourselves and organise a time for an initial consultation with our physiotherapist.

  2. First Appointment: Our physiotherapist will visit you at home for the first appointment and listen carefully to your concerns, working together to establish your mobility, strength, balance and fitness related goals.

  3. Assessment: Over the first 1-2 appointments your physiotherapist will complete an assessment of your mobility, strength, balance, fitness and/or injury. Assessment will involve gathering subjective information through a chat with you or a support person, and objective assessment through a range of outcome measures and observation.

  4. Your Health Plan: We create a written plan, detailing how we plan to achieve the agreed upon goals. We will share this with you and your HCP manager.

  5. Getting started: Your physiotherapist will provide evidence-based therapy, guiding you through their recommendations and exercises.

  6. Progress Tracking and Adjustments: We regularly monitor and assess your progress, making adjustments to the care plan as needed for you to achieve your goals.
